A preliminary report for final month’s Air India crash that killed 241 individuals on board — with a sole survivor miraculously strolling away — discovered that the airplane’s engine gasoline cutoff swap was flipped simply three seconds after taking off.

It is unclear how or why the swap was flipped, which ceased all gasoline to the engines and shortly pressured the London-bound Boeing 787 Dreamliner to lose its thrust and crash.
